Using an internet cafe or say, free wifi in a public place without using a VPN, could be asking for a MITM scenario. The MAC spoofing is a good idea though... I tried Technitum MAC Changer a while back & seemed good/easy to use.
I guess with anything.... incl the VPN providers... if you don't know the people you're dealing with (VPN=honeypot? Net cafe owner=criminal and/or keylogger present?) there's some risk to using any/all.
